MAILABLE FORM

• Honor your hero (In Honor) or memorialize one who has passed (In Memory)
• Email requested information and photo to aaron@midwesthonorflight.org by August 15th
  • Provide the rank/name, branch of service, and era 
    • In honor can include dates of service (example below)
    • In memory can include date of birth to date of death (example below)
  • Provide a photo (optional) - branch logo will be used in place of photo if not provided/desired
• Sign will be placed somewhere throughout the course on the day of the event
• Donor can take home at the end of the tournament (we can also arrange for pick up if unavailable that day, shipping is an option as well but the size of the sign may be a significant cost)
